The Leanne Fund Launches New 'Transition Packs'

The Leanne Fund is proud to announce the launch of a new initiative - Transition Packs - designed to support young people as they move from paediatric to adult care within Cystic Fibrosis (CF) services.
Transitioning to adult care is a significant milestone for young people living with CF, and it can often be an overwhelming time. The Leanne Fund recognises the emotional and practical challenges involved in this journey and remains committed to walking alongside individuals every step of the way.
Transition Packs are a thoughtful new addition to our support services - a tangible reminder that The Leanne Fund continues to be there as a constant source of support, even as care teams change. Each pack is filled with useful, comforting items and information to help ease this transition and reassure young people that they are not alone.
The first pack was recently gifted at Aberdeen Royal Infirmary by Kerry, CF Assistant Practitioner with NHS Grampian, whose vision and dedication were key to the creation of this service. We are deeply grateful to Kerry and the entire Grampian CF team for their enthusiasm and collaboration, which enabled us to pilot the Transition Packs in their service.
This initiative aligns with The Leanne Fund’s mission to provide person-centred support to individuals and families affected by Cystic Fibrosis across the country. By working closely with CF clinical teams, we ensure that our services continue seamlessly into adult care – adapting to each individual's changing needs.
With the success of this pilot, we are excited to explore opportunities to expand the Transition Pack initiative to other regions, ensuring more young people receive the reassurance and support they deserve during this key stage of their CF journey.
