Moving from paediatric to adult CF care is a big step, with a new team and a new chapter in their care. Our Transition Packs are a little reminder that, while your care team may be changing, our support isn’t.

 

We want every young person to know that the Leanne Fund is still here for them -supporting, encouraging and standing by them, just as we always have.

 

Our Transition Packs have been devised in collaboration with Kerry, CF Assistant Practitioner and supported by the NHS Grampian team, to help make this important transition feel a little easier.
